We are pleased to offer the ability to customize many of our product lines. Here you can learn the requirements for placing a custom order as well as detailed instructions for placing your order.

Although we are not able to take your custom order online, we are working hard to provide a capability to allow you to customize products online, which we hope to introduce next year.
1. Select Product:
     
How To Place a Custom Order

Note: Presently, all orders for custom products must be placed via fax or email using a Prevention Partners order form.

  1. Complete an order form
    You can find our order form in the Prevention Partners catalog.  If you do not have a catalog, you may request one here OR you can get one online here.

    1. Complete the Bill To: section (labeled step 1 on the form) and the Deliver to: section (labeled step 5 on the form), paying close attention to provide a valid and accurate name, phone number, fax number and email address.

    2. Complete the Method of Payment: section (labeled step 2 on the form), making sure to select the appropriate method of payment for you or your organization.

    3. Proceed to the Custom Product section of the order form (part of the section labeled as step 4) and identify the item that you would like to personalize.   You may start with one of our designs and add your school or organization name or some other text that is meaningful to you; or, you may start with our slogan and add your logo; or, you may use your own design.

      For example, if you wanted to personalize our "Celebrate Red Ribbon Week" plastic wristband with the name of your organization below our slogan, you would enter DPW009 in the first column labeled Custom Product, the quantity in the second column, and the name of your organization in the Personalization column. Be sure to specify the product color and imprint color.

      When entering the quantity and unit cost on the order form, be sure to use the 'Select Product' guide above to confirm the quantity requirements and pricing for your custom product. If no pricing is provided, please call 800.394.1213 for pricing.
      As another example, if you wanted to add your logo to our "Celebrate Red Ribbon Week" stadium cup, you would enter DSC009 in the first column, the quantity in the second column, and "artwork emailed" in the Personalization column. Be sure to specify product color and imprint color.

      As a last example, if you wanted to do a unique design, you would simply enter the quantity in the second column and the "artwork emailed" in the "Personalization" column.

      For any text that you request to be included, be sure to use the desired punctuation and capitalization.

      If you need to send us artwork, please confirm that your artwork conforms to our specifications.   If so, please email your artwork to artwork@preventionpartners.com.

    4. Complete the Signature: section by printing and signing your name.


  2. Fax the order form to us at 800.237.2149 OR you can mail it to us.

  3. Artwork Layout
    After we receive your order and artwork, if applicable, we will layout the design on a template. We will fax a copy of the artwork to you for your approval on our Artwork Approval form.

  4. Approve Artwork
    If the design meets your requirements, sign the Artwork Approval form and fax it back to us at 800.237.2149.

    If the design does not meet your requirements, please mark any changes on the Artwork Approval form and fax it back to us at 800.237.2149. We will make the necessary modifications and send you a new Artwork Approval form for your review and approval. If the new design meets your requirement, sign the Artwork Approval form and fax it to us at 800.237.2149.

    Note:Two changes to the design prior to your approval are allowed.   Additional changes may incur additional cost.

  5. Production
    Once we have a signed order form and a signed Artwork Approval form, the order will go to production. Allow up to 20 business days from this point to receive your order.
